Certified Diabetes Educator, Family Practice

Job Responsibilities:

A Certified Diabetes Educator (CDE) is a health professional who possesses comprehensive knowledge of and experience in diabetes prevention, prediabetes, and diabetes management. Certified Diabetes Educators educate, support, and advocate for people affected by diabetes, addressing the stages of diabetes throughout the lifespan. The CDE empowers people with diabetes to engage in effective self-management to achieve individualized behavioral and treatment goals that reduce risks and optimize health outcomes.


• Promote the mission, vision and values of the organization
• Document all clinical data correctly and appropriately in patient’s EHR
• Communicate patient information after consultation with provider protocol and/or provider orders
• Management of high-intensity, insulin pump and continuous glucose monitor-related telephone encounters
• Excellent professional judgment and critical thinking
• Demonstrate excellent communication skills via phone and in-person with patients, care givers, InterMed staff, and outside staff
• Work collaboratively with all team members
• Demonstrate initiative, responsibility, flexibility, and appropriate decision making
• Advanced understanding of continuous glucose monitoring and insulin pump models, use, download process, and interpretation of data preferred
• Able to safely and correctly perform continuous glucose monitoring trials, at the direction of the provider, including education to patients, insertion of sensor, removal of sensor, and preparation of downloaded data
• Advanced understanding in the disease processes of and in working with people who have type I, type II, LADA, and gestational diabetes
• Serve as diabetes resource to other staff
• Ability to prioritize and handle multiple tasks at the same time
• Provide education to patients and on the diabetes disease process, taking medication, including injectables, health eating, risk reduction, blood glucose monitoring, problem solving, being active, and healthy coping.
• Assist the diabetes specialists in the development and expansion of the diabetes program from its current format to a fully-integrated model throughout InterMed
• Comfortably provide direction on medication adjustment, including insulin, with the approval of supervising provider
• Assess, monitor, and support treatment plans for patients at various stages of the disease
• Accurate and thorough documentation in diabetes nurse visits notes
• Refill patient medications according to protocol per provider approval and oversight
• Oversight of clinical environment with attention to safety and infection control
• Maintain confidentiality and compliance in all patient care activities
• Participate in continuing and professional development as required by registered nurse license and diabetes educator certification
• Administer routine and non-routine medications per standing order
• Provide on-going assessment and recommendations to the treatment team and patient/family for adjustments in the plan of care as required
• Assist providers with patient care when requested
• Special projects as assigned by coordinator or manager

Job Qualifications:
• Graduate from an accredited school of nursing
• Unencumbered, active registered nurse licensure in state of Maine
• Certified as a Diabetes Educator (CDE) by the National Certification Board for Diabetes Educators (NCBDE)
• Previous experience working in similar environment
• Professional appearance and demeanor
• Proficient in Windows applications, electronic medical record preferred

Other Job Information (if applicable):
40 hours a week. Monday-Friday 8:30am to 5:00pm.
